Role Description

OSH’s Transitional Care program supports multidisciplinary care across facility-based care teams, OSH care teams, utilization management, care navigation, and patients and caregivers.

The Analyst, Transitions will work closely with the OSH Transitions of Care Program leaders and OSH field team to drive key initiatives related to supporting patients and improving operational performance. Additionally, the Analyst will collaborate with the broader Population Health team at Oak Street to connect operational activities to business impact. The Analyst will own strategic initiatives, partner with senior leaders, and drive operational workflow improvements.

The Analyst, Patient Engagement will also be part of Oak Street Health’s immersive Analyst Development Program (ADP), a multi-year learning experience designed to build core problem-solving, data analysis, communication, and project management skills. This role will consist of small and large group training, exposure to senior leadership, and the opportunity for quick career progression and collaboration across many business functions.

Core Responsibilities

  • Drive strategic analyses, develop data-driven insights, and own reporting for the Transitions of Care program

    • Build and analyze large-scale datasets to identify trends, connect operational action to financial impact, and surface key insights for improving patient growth and engagement

    • Communicate analytical insights clearly across various levels of the organization, from regional leadership to the Chief Population Health Officer

    • Collaborate with cross-functional leaders to uplevel existing workflows and/or develop new processes (e.g., centralizing PDV holds)

    • Develop and maintain high-visibility tools and dashboard (e.g., transitional care dashboard) to enable field and central leaders to better understand growth and recovery efforts

    • Serve as the project manager and analytical engine for pilots and new initiatives

    • Collaborate with Transitions program leaders to develop and iterate on new processes (e.g., WC Involvement in PDV Scheduling)

  • Professional Growth & Development

    • Actively participate in small and large group trainings to develop technical and leadership skills

    • Seek feedback and mentorship from peers and senior colleagues and participate in career development planning.

  • Other duties as assigned

What are we looking for?

  • High level of organization and detail orientation

  • Experience collaborating on projects across multiple departments

  • Proficient with Microsoft Excel or other standard analysis tools (SQL, Alteryx, and/or PowerBI a plus)

  • Strong intellectual curiosity and problem-solving ability

  • Ability to navigate leadership and operational complexities

  • Passion for excellence, willing to go the extra mile to achieve objectives and ensure consistently high quality work product

  • Quick learner comfortable with ambiguity

  • Strong accountability and discipline against timelines and deliverables

  • Bachelor’s degree required

Pay Range

The typical pay range for this role is:

$43,888.00 - $93,574.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
