Role Description

At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. The Associate, Population Health reports to the Manager of Population Health and is responsible for supporting chronic condition management and admission prevention strategy efforts, including, but not limited to, programs designed to improve clinical outcomes for patients with heart failure and diabetes.

The ideal candidate is a self-starter, enjoys solving challenging problems, uses data to drive insights, and excels in a fast-paced environment. This will be a high-visibility role requiring cross-team collaboration within Population Health and other corporate teams (e.g., Managed Care Operations, Business Intelligence, Clinical Quality etc.), partnership with clinical and operational field leaders, and regular communication with senior leadership.

Core Responsibilities:

Under direction of the Manager of Population Health and in partnership with range of critical stakeholders, including field and executive leaders:

  • Business Analytics and Reporting: Create and maintain operational and outcomes reporting for chronic condition management programs; Perform analyses of high-risk patient populations and program performance to identify opportunities for improvement and additional business impact.

  • Project Management: Oversee day-to-day of program operations to ensure performance. Construct detailed project plans, set timelines, update key stakeholders, and ensure that projects are completed on time and achieve expected outcomes.

  • Program Design / Implementation: Collaborate with cross-functional teams, including Medical Directors, Clinical Operations, Product, Data, and other Population Health leaders, to design and implement novel programs to drive clinical outcomes and keep patients healthy, happy, and out of the hospital.

  • Stakeholder Management: Collaborate with cross-functional teams to drive program performance.

  • Communication: Strong verbal and written communication skills to convey updates to field leaders, care teams, and leadership, including status reports, dashboards, and presentations to all stakeholders.

  • Other duties as assigned.

What we’re looking for

  • 1+ years of consulting, business analytics, and/or other applicable experience in healthcare

  • Analytical and data savvy

    • Highly proficient with Microsoft Excel, experience working with large datasets

    • Experience/proficiency in SQL is preferred

    • Ability to succinctly summarize and synthesize large amounts of information

    • Detail-oriented with excellent problem-solving abilities

  • Strong relationship-building skills to foster collaboration with key stakeholders

    • Strong written and verbal communication skills; highly proficient with Microsoft PowerPoint

    • Strong interpersonal skills that support effective stakeholder engagement

  • Someone who is a self-starter

    • Ability to work independently and manage multiple projects simultaneously

    • Strong accountability and discipline against timelines and deliverables

    • Passion for excellence, willing to go the extra mile to achieve objectives and ensure consistently high quality work product

    • Quick learner and someone who is comfortable with a high level of ambiguity

  • Comfortable navigating ambiguity

  • Bachelor degree required

Pay Range

The typical pay range for this role is:

$43,888.00 - $93,574.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
