Location: Dallas, Ellis, Navarro, Kaufman, Rockwell, Hunt, Collin and Tarrant Counties in Texas

Position Summary

This role is a field-based position, with an office at home. This position will require travel to meet in person with members around Tarrant/Dallas and Surrounding Counties 25% of the time.
Hours for this role would Monday-Friday 8am-5pm.


Candidate is responsible for conducting ongoing transition planning starting when the member turns 15 years old. The Transition Specialist will assure that teens and young adult members receive early and comprehensive transition planning to help prepare them for service and benefit changes that will occur following their 21st birthday. The Transition Specialist will use a team approach with the member, others in the member’s network, and the member’s service coordinator. The Transition Specialist is dedicated to counseling and educating members and others in the member’s support network about considerations and resources for transitioning out of STAR Kids. The Transition Specialist will create a continuity of care plan specific to the member and member’s needs to ensure a successful transition to adult care. Consults with supervisor and others in overcoming barriers in meeting goals and objectives, presents cases at case conferences, utilizes interviewing skills to ensure maximum member engagement based on key questions and conversation.

Required Qualifications

- Must be willing and able to travel 25% in Tarrant/Dallas and surrounding counties. Reliable transportation required. Mileage is reimbursed per our company expense reimbursement policy.

-1 year of Service Coordinator experience required.

Preferred Qualifications

- Crisis intervention skills
- Managed care/utilization review experience
- Case management and discharge planning experience

-Pediatric to adult care planning experience

-StarKids Experience

Education

Minimum of a Bachelor's degree in Behavioral/Mental Health or related field

Pay Range

The typical pay range for this role is:

$21.10 - $40.90

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
