Case Management Analyst – LTSS (Virginia – Field Based)

Aetna’s Long‑Term Services and Supports (LTSS) program is looking for a passionate, member‑focused Case Management Analyst to support individuals and families across the Northern Virginia region. This is a field‑based role where you’ll spend meaningful time face‑to‑face with members—listening, advocating, and helping them navigate complex health and social needs.

What You’ll Do
Build trusted relationships with members through in‑person visits (approximately 70% travel).
Conduct comprehensive assessments using clinical judgment and critical thinking to understand medical, behavioral, and social needs.
Develop and implement individualized care plans that promote health, independence, and quality of life.
Collaborate closely with providers, community resources, and interdisciplinary teams to ensure coordinated, high‑quality care.
Connect members to services, supports, and resources that align with their goals.
Ensure care delivery meets regulatory requirements and Aetna quality standards.
Advocate for medical appropriateness while supporting positive member engagement and outcomes.
Required Qualifications
2+ years of experience in behavioral health, social services, or a related field aligned with program focus.
Residency in one of the following Virginia areas: Alexandria, Arlington, Vienna, McLean, Woodbridge, and/or Springfield.
Reliable transportation and the ability to travel up to 70% of the time (mileage reimbursed per company policy).
Preferred Qualifications
Experience in case management or discharge planning.
Managed Care experience.
Education
Bachelor’s degree or non‑licensed master’s degree required.
Degree in behavioral health or human services strongly preferred.

The typical pay range for this role is:
$21.10 - $40.90This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
