Position Summary
We are seeking a compassionate and detail-oriented professional to support our case management team in delivering high-quality, member-centered care. In this role, you will apply critical thinking and sound judgment to collaborate across disciplines, helping guide the case management process and ensure the best possible healthcare outcomes for our members.
You will play a key role in coordinating care, providing education and support, and connecting members to the resources they need. By leveraging care management tools, data insights, and a holistic approach, you will assess member needs, identify risks, and develop effective plans that promote improved health and well-being.
Working closely with clinical teams, leadership, and external partners, you will help remove barriers to care, advocate for appropriate services, and encourage member engagement through coaching and motivational support. Your ability to empower individuals to make informed healthcare decisions will be essential in driving meaningful, lasting outcomes.
This position also involves ongoing monitoring and evaluation of care plans, ensuring compliance with regulatory standards and organizational guidelines, while continuously striving to enhance the quality and appropriateness of care delivered.
Required Qualifications
Candidate must reside in Vienna, Alexandria, Arlington, Annandale, Mclean, Sterling or Reston, VA or surrounding areas (within 15 minutes)
Must possess reliable transportation and be willing and able to travel up to 75% of the time in the Northern Virginia region. Mileage is reimbursed per our company expense reimbursement policy
- 2-4 years of experience in behavioral health, social services or appropriate related field equivalent to program focus
Must be comfortable working in a fast-paced environment
Preferred Qualifications
- Bilingual
- Experience with home visitation
Education
- Bachelor's Degree or non-licensed master level clinician required, with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling)

The typical pay range for this role is:
$21.10 - $40.90This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
