Role Description:
At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care.
The Population Health Sr. Analyst will be instrumental in designing and implementing key Population Health priorities, with a focus on our Transitions of Care programs. The Population Health Sr. Analyst will work on programs and interventions, targeted to the right patients, to improve quality and cost of care. They will be an effective cross team leader, working side by side with Treehouse members and collaborating with providers, care teams, operations team members.
Content areas for the Sr. Analyst will include, but not be limited to, admission prevention, transitions-of-care (readmission prevention), HEDIS / Stars, preferred partner relationships and medical cost management. This will require building relationships with our Treehouse team (including Data Science, Product, and Finance) and clinicians, including primary care physicians, nurse practitioners, pharmacists, registered nurses, licensed clinical social workers, and others.
This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently across Oak Street divisions, regions, and centers.
Core Responsibilities:
Support the execution and the ongoing refinement of Population Health programs including workflow optimization in Epic.
Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with Transitions of Care program strategy and design.
Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities.
Lead communication and content development related to key population health metrics, including quality measures, utilization measures, and financial measures.
Manage admission notification strategy in partnership with Product and Data engineering, including vendor management.
Other duties, as assigned.
Required Qualifications
At least 2 years of experience in healthcare, consulting, or business operations.
Preferred Qualifications
Experience in care delivery / care management settings or at a value-based care organization.

The typical pay range for this role is:
$46,988.00 - $112,200.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
